How much does it cost to sign up?

Medicare covers remote patient monitoring and chronic care management for eligible patients. We bill Medicare directly for all covered services.

Do I have to wear anything or do any extra work?

Most of our patients get a sleep mat which goes under the mattress to passively collect resting heart rate and respiratory rate. Our team will set up the device for you, and we take care of any issues. Some patients with certain circumstances may need other devices - someone with hypertension may have their physician order a blood pressure cuff, for example.

Will the monitor interfere with my pacemaker monitor or other devices?

No. The sleep mat that goes under your mattress works as a very sensitive pressure plate to pick up micro vibrations referred to as ballistocardiography. No frequencies or radio waves are emitted from the device.

How long do services last?

Unlike Home Health services, our service is not limited to a set number of days. As long as you have ongoing chronic conditions, you qualify for our life-changing services.

What other benefits do you offer aside from monitoring?

Your assigned Patient Care Coordinator can help escalate concerns with your provider or specialist. They will help coordinate your ongoing needs and concerns to make sure you don’t fall through the cracks! They can also help arrange senior transportation, meals on wheels, or help you find what you need to make sure you are taken care of!
