How much does it cost to sign up?
Nothing. We provide the devices at no cost to you, ever. If the services are billed under your NPI, then you only pay Proactive Health AFTER you have been paid by insurance or otherwise.
What additional work do we need to do?
Very minimal - send the referral to Proactive Health, and we will take it from there - scheduling the setup, installing the devices in the patient’s home, and the ongoing monitoring and care coordination. We can even handle the billing on your behalf if you are billing under your NPI.
How does this help my patients?
Our monitoring system, meshed with our responsive Patient Care Coordinators, will be able to help escalate minor problems in a timely manner. This allows patients to be seen sooner, preventing small problems/complications from becoming major, life-threatening issues.
As a physician, how does this help my practice?
Proactive Health provides additional care to your patients and drives incremental revenue for you through Medicare CPT codes.
As a home health agency, how does this help me?
Our monitoring will be with the patient every day, even when you may not have a scheduled appointment with the patient. By partnering with Proactive Health, we can help you reduce your hospital readmission rates. The monitoring also continues after the patient discharges from home health. When a qualifying event occurs, we can work with you to readmit the patient to home health services.
