Covered By Insurance:

You have been prequalified for this service through your insurance who works with us as a preventative measure. We will handle the billing and paperwork to ensure your services are properly setup.

What's Included:

  • Daily monitoring of your health data

  • Alerts to your care team if concerning changes arise

  • Support from a dedicated Patient Care Coordinator

  • Communication with your primary doctors and providers

  • Assistance with, and coordination of, any doctor’s visits

  • Setting up any additional care

  • Faxing of your reports

  • Device maintenance and tech support

  • Other senior resources

Length of Service

Insurance will cover this for life. It is recommended that you stay on the service for at least 1 year, however we encourage you to stay on for life to get the full benefits.

 

If you choose to stop or pause the service at any time, tell your Patient Care Coordinator (PCC).

FAQs

Why am I being monitored?

You have been recommended for our services by your current providers. This may be to monitor existing chronic conditions, track progress in your health, or to work as a preventative measure to stay ahead of health concerns. No matter the reason, our monitoring is in place to prevent unnecessary hospital visits.

How will I know if my device stops working?

Your PCC will recognize if the device stops collecting data. In that instance, they will reach out to you to try and fix the problem remotely. If the problem is not resolved, a member of our tech support team will then come out and get the device working properly.

What if there is something that looks concerning?

If your PCC notices a change in your vitals, they will reach out to you to discuss any potential changes or symptoms you are experiencing. If needed, they will then schedule any necessary appointments for you.

Do I still need to see my regular doctor?

Yes, you will still need to attend all of your regular appointments. We work in coordination with your current care team, but we do not replace your primary care.

What will this cost me?

Good news, you are prequalified! Our service is covered by your insurance, therefore we will work with them to handle payment.
